Pre-conference workshop – Effective Learning from Incidents

This highly interactive workshop will look at two specific well completion, well workover, intervention well control incidents within the industry.  We will cover the complete lifecycle of the incident from well concept through to the ultimate investigation, looking closely at what:

  • Sequence of events and loss of barriers led to the well control incident 
  • Well design and completion methods can contribute to a well control incident 
  • Human factors play a contributory part in the well control incident 

Working in teams, we will facilitate the sharing of industry incident information, providing a real learning opportunity from those incidents. We will use an incident investigation and analysis method which delegates can learn from and can apply in their own organisation.    

The exercises in this workshop can show what is possible to recreate when delivering  IWCF Level 5 and enhanced programme case studies.  CPD points are also applicable. 

This workshop will ultimately show us how to design out future incidents.

Target audience:

Well engineers; Coiled Tubing/Intervention engineers/supervisors; Equipment manufacturers; senior rig crew involved in workover and well abandonment operations; HSE inspectors

3.15pm – 4.00pm: Wrap up and final networking
